Use of a handover instrument in an adult intensive care unit: experience report

The handover is a communication activity, which requires standardization of information to be effective. In the hospital environment, the care process requires qualified estimates for patient safety. In view of this, the objective is to highlight the voice of standardization methods for the passage of planting. This is a descriptive, qualitative study that reports an experience on the Supervised Practical Classes of the Discipline of Nursing and the Critical Patient in one of the ICUs of the University Hospital of Western Paraná (HUOP). After observational analysis to get to know the infrastructure and adapt to the dynamics of the care provided, the shift change is recommended to ensure the continuity of care, through checklist instruments that guide clear and objective communication about the intercurrences of the shift and notes about the clinical picture of the patient, offered to the team and positive clinical evolution for the patient. With the practical activities carried out, there was greater consolidation of the theoretical foundation, understanding the sacred role of nursing in executing managerial skills and the systematization of care with a macro view, considering the singularities that make up an ICU.
