HIV/AIDS, marginalization, and life expectancy of the transgender population: an integrative review

This review highlighted the impact of HIV/AIDS on life expectancy and the marginalization of the transgender population. The search was conducted in LILACS and MEDLINE databases between April and december 2025, following the PRISMA protocol. Studies published between 2020 and 2025 in Portuguese, English, and Spanish addressing HIV/AIDS, discrimination, life expectancy, and access to health care among trans people. Findings reveal vulnerabilities resulting from structural, social, psychological, and programmatic factors. Gender discrimination reduced access to medical consultations and HIV testing. Trans women have up to 34 times higher risk of infection compared to the general population. Stigma, socioeconomic inequalities, and health system failures compromise treatment adherence. Mental health problems, such as depression, suicidal ideation, and substance use, further increase vulnerability. Early exposure in adolescents underscores the need for specific prevention strategies. Digital interventions showed potential to expand testing and prevention. HIV/AIDS is also a reflection of structural inequalities affecting trans lives. Reducing mortality requires inclusive policies, combined prevention, mental health support, and recognition of this agenda as an ethical and political priority.
